your dental and medical history and the reason for your visit,
then give you a comprehensive exam, including x-rays, to assure
that you receive the exact treatment that meets your dental needs.
In addition, a hygienist will determine a personal cleaning program
to maintain your dental health.
To save time on your visit, please bring:
1. Dental insurance card (if you have coverage)
2. Photo ID
3. Information sheet, download at right |

If you are unable to fill
out the information form and agreement beforehand, please plan
to arrive at least 15 minutes before your appointment time.
Either way, don't worry. Our friendly staff will
make sure you leave your first visit to Smart Smart happy and
